Pre-Register For A Library Card

Do not use this form if you lost your library card. Bring your photo ID and $5 to the library to get a replacement.

After completing this pre-registration, please bring proof of identity, residency, and mailing address to the library in order to finalize your application. A list of acceptable items for ID/residence/address is available on our Library Card Registration Policy.

By submitting this pre-registration, you acknowledge that you have read and understand Homer Public Library's policies, meet eligibility requirements, and accept full responsibility for the use of your library account.

A parent or guardian must apply for a juvenile (under 18 years of age) in person.
